In general the point-slope form for a line through a point #(hatx,haty)# with a slope of #m# is
#color(white)("XXXXX")##(y-haty)=m(x-hatx)#

For the given point (#1,3)# and slope #4#, this becomes
#color(white)("XXXXX")##(y-3) = 4(x-1)#

While the above is a complete solution to the question, you might want to write this as
#color(white)("XXXXX")##y = 4x -1#
#color(white)("XXxxxXXX")#or
#color(white)("XXXXX")##4x+y = 1#
