No, there are exceptions.
Explanation:
Who once said "We have nothing to fear but fear itself"? It was Franklin Roosevelt, and he wasn't asking a question. Hence, the outside-the-quotes placement.. Of course, if I am quoting an actual question, the question mark belongs inside the quotation marks.
