Could a wormhole be caused in space by somehow placing planets too closely together thus wearing out and eventually tearing the space between planets?

Extremely unlikely.
Explanation:
The gravity between two planets is way too weak. The vicinity of a strongly gravitating object that produces a great deal of space-time curvature, such as a black hole, is a more likely candidate.
